A panel may be formed as a result of this campaign for Grade VII – Senior Executive Officer from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.
Purpose of the Post Case management is a system for the planning, arranging and subsequent co-ordination and delivery of services using individually tailored supports within available resources whilst having a person-centred approach and holistic focus.
The initial purpose of this post is to appraise options and arrange appropriate service provision for Children with a disability in consultation with the person/family, Children's Disability Network Team and Disability Operations Team.
The Case Manager may act as the key contact for the individual with complex needs.

Details of Service This position is for Case Manager for Children in Disability Services in South Tipperary
Children and Families who require additional community based supports are referred to the Case Manager who will then link with the Child/families and explore what supports are available and can be offered. Some children require a short targeted piece of co-ordination and other children, may require ongoing significant liaison with service providers and families.
This can include attendance on home visits, school meetings, professional meetings etc. Regular communications with local clinicians is critical with opportunities to liaise with specialist services as appropriate. The Case Manager will identify current trends and needs and assist with the development of community services to meet those needs by liaising with local providers in relation to service development.
