Performance management systems are part of role and you will be required to participate in the Group's performance management programme Clinical Coding Educate staff on and be an advocate for ABF within the hospital so that the importance of ABF and its effect on the hospital budget is known by all. Consistently be part of raising awareness throughout the hospital of the importance of clear and concise Clinical Information being recorded. Obtain patient medical records for Inpatient and day case discharges which have not been entered onto the HIPE system. Extract the Clinical Information regarding diagnoses and procedures recorded in the patient medical records in accordance with Irish Coding Standards (ICS) in the first instance and Australian Coding Standards (ACS). Codify the diagnoses and procedures using the current classification system according to the rules of ICS & ACS. Enter the coded information onto the HIPE database. Validate and check own work at regular intervals to ensure completeness and accuracy using, as appropriate, relevant ICT tools including CHECKER & PICQ to assist in this duty. Work as a team member to ensure that all discharges are coded within the given timelines and that the department can meet its coding deadlines. Encourage and support a team approach to coding within the HIPE Department. Be able to identify information gaps in clinical information for common admission types and treatment types. Liaise with Senior Coders / Departmental Manager / Clinical Staff regarding information gaps to ensure that the information recorded on HIPE is specific and true to the care the patient received. Ensure that the Administrative Information on the HIPE system is specific and true to the episode of care being coded. Maintain knowledge of existing electronic systems pertinent to HIPE to ensure the accuracy of the data recorded on HIPE. Responsible for the coding of patient discharges in line with requirements/deadlines as laid down by the Healthcare Pricing Office (HPO) and the DoHC and the HIPE Manager. Ensuring that patients' record charts are coded consistently and accurately in timely manner. Responsibility to ensure that the confidentiality of patient data is maintained at all times. Knowledge of how daily download from PAS are carried out and have the ability to carry out this function. Knowledge of how the monthly export of coded HIPE data to the HPO is managed on a monthly basis. Responsible for liaison with the IT Department to ensure efficient operation of the HIPE software. Provision of ad hoc reports as and when required. Embrace and learn the use of new systems pertinent to HIPE as they come on-stream. Work in locations (whether ward-based or office-based) as directed by management in order to maximise the efficient collection of coding information. Track location of patient medical records appropriately according to the Hospital Policy. Maintain and update coding knowledge and attend relevant education activities. Once proficient in coding to assist in the training of new coders and to act as a mentor to new coders Correcting all coding queries returned by the HPO, & PICQ in a timely fashion Run CHECKER on HIPE Portal as required & always before an Export. Be open to learning new elements of the HIPE system as they arise. Provide cross cover during holiday and or other leave in the Department Embrace change and be willing to adopt new work practices Support the HIPE Manager in creating/promoting open communications and healthy working relationships within the HIPE Department. All circulars relating to coding practices to be read and any recommendations to be implemented Liaise with all and any Hospital staff, both Administrative and Clinical and with the Healthcare Pricing Office as required to ensure the complete and accurate recording of data on HIPE and to share Clinical Coding and ABF information with the Administrative and Clinical staff in the hospital. Know how to run patient and discharge reports from the HIPE system. Any other duties with regard to the office as may be assigned from time to time. KPI's The identification and development of Key Performance Indicators (KPIs) which are congruent with the Hospital's service plan targets. The development of Action Plans to address KPI targets. Driving and promoting a Performance Management culture. In conjunction with line manager assist in the development of a Performance Management system for your profession. The management and delivery of KPIs as a routine and core business objective. PLEASE NOTE THE FOLLOWING GENERAL CONDITIONS: Employees must attend fire lectures periodically and must observe fire orders. All accidents within the Department must be reported immediately. Infection Control Policies must be adhered to. In line with the Safety, Health and Welfare at Work Acts 2005 and 2010 all staff must comply with all safety regulations and audits. In line with the Public Health (Tobacco) (Amendment) Act 2004, smoking within the Hospital Buildings is not permitted. Hospital uniform code must be adhered to. Provide information that meets the need of Senior Management. To support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service. Risk Management, Infection Control, Hygiene Services and Health & Safety The management of Risk, Infection Control, Hygiene Services and Health & Safety is the responsibility of everyone and will be achieved within a progressive, honest and open environment. The post holder must be familiar with the necessary education, training and support to enable them to meet this responsibility. The post holder is responsible for ensuring that they comply with hygiene services requirements in your area of responsibility.
Hygiene Services incorporates environment and facilities, hand hygiene, catering, cleaning, the management of laundry, waste, sharps and equipment. The post holder must foster and support a quality improvement culture through-out your area of responsibility in relation to hygiene services. The post holders' responsibility for Quality & Risk Management, Hygiene Services and Health & Safety will be clarified to you in the induction process and by your line manager. The post holder must take reasonable care for his or her own actions and the effect that these may have upon the safety of others. The post holder must cooperate with management, attend Health & Safety related training and not undertake any task for which they have not been authorised and adequately trained. The post holder is required to bring to the attention of a responsible person any perceived shortcoming in our safety arrangements or any defects in work equipment. It is the post holder's responsibility to be aware of and comply with the HSE Health Care Records Management/Integrated Discharge Planning (HCRM / IDP) Code of Practice.
