We are looking for a Community Support Worker to join our team.
Job PurposeThe purpose of the job is to promote and support the rights of service users to access day service and community integration opportunities.
The service users will achieve community integration through personal development, valued social roles and independent living skills in their Day Service and their local community.
To assist the Community Services Manager in the development, implementation, monitoring and delivery of a quality customer-focused, cost-effective community service to clients at an appropriate time and setting.
Minimum RequirementsEssential: A minimum of 1 year's experience working in the field of disability.Essential: A minimum of QQI level 5 in Health & Social Care or equivalent.Willingness to participate in RehabCare mandatory and further development training programme.Experience of working 1:1 in the community.Job Duties & ResponsibilitiesTo ensure all interaction with service users is approached with dignity, respect, equality and incorporates choice.To develop and deliver person-centred programmes to meet the needs of service users as identified through Person Centred Planning and to maintain the required records.To ensure best practice in line with up-to-date research regarding Quality of Life improvements.To support individuals in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.To enshrine the concept of independent living by supporting clients to live in their own homes independently, integrate into their local community, develop valued social roles and access local resources.Additional RequirementsFull Clean Driving Licence and access to a car for business purposes.To promote and be actively involved in teamwork at all times.To liaise with families, the multidisciplinary team and other personnel as directed by the Community Services Manager.A flexible shift system will operate.BenefitsCompany Pension SchemeEducational leave27 days annual leaveBike to work schemeIncome protectionUnpaid leaveHealth and Wellbeing ProgrammeDigital DoctorFinancial support for studying and study leaveLong Service Reward SchemeSick leaveEmployee Assistance Programme (EAP)*Rewarding Career *Career with meaning *Attractive benefits *Work-life balance* To view the full extensive job description please, click apply to visit our company site and download the attachment at the bottom of the page.
