Our work positively impacts the lives of over 10,000 people with disabilities, equipping them with the confidence, skills, and self-belief to achieve their expressed goals Community Support Worker Galway Outreach & Community Support Services, Galway City & County, County Galway Permanent Contract, 35.5 hours Job Purpose This is a service aimed at providing a range of supports to School Leavers and RT leavers between the ages of 18 and 25 years old.
The purpose of the job is to promote and support independent living and community integration, personal development, social interactions and work opportunities for people with Autism.
Additional positions likewise have the purpose to support these areas for people with Autism, intellectual disabilities, physical and sensory disabilities.
To assist in the delivery of individualised community support packages to a number of clients with intellectual, physical and or mental health disability some of whom may present with behaviors' that challenge .
This includes the provision of individual programmes and activities to meet the needs, choices and abilities of service-users.
Minimum Education & Skills required QQI level 5 in Health & Social Care or equivalent.
Two years' experience is desirable in the provision of community and service based programmes and activities to promote independence and meet the needs, choices and abilities of individuals.
Candidates should have a comprehensive understanding, knowledge and experience of working with people with neurological conditions and be able to maintain a high standard of care in line with RehabCare's policy and procedures Job Duties & Responsibilities To support service users to carry out rehabilitation programmes for physical, cognitive and emotional impairment to maximise quality of life To liaise with families, multidisciplinary teams and other personnel, as directed by the management team, to ensure the best quality of service with the individuals as directed by community services manager To document information and complete paperwork as deemed necessary for delivery of service to the individuals To adhere to and ensure the required standards in line with HIQA regulations and reporting to HIQA is maintained Additional requirements Full Clean Driving Licence and access to a car for business purposes is required To promote and be actively involved in teamwork at all times.
To liaise with families, the multidisciplinary team and other personnel as directed by the Community Services Manager To maintain a basic knowledge of first aid/personal care and to implement good hygiene practices.
