We are seeking Community Support Workers - New Directions and Outreach for Day Services based in Clare, Kerry and Kildare.
As part of your role, you will:Empower Service Users to live independent lives in their home and the community.Support the development and delivery of the person-centred planDevelop community partnerships to foster active citizenship in line with service user goals and wishes.Transport service users to access the community.Be responsible for Key Worker duties i.e.
