Dncc2024097 Community Healthcare Network Gp Lead (Líonra Cúram Sláinte Pobail, Ceannaire Na Ndochtúirí Ginearálta)

A key component of the Sláintecare vision of providing the 'right care, in the right place' at the right time' is the development of 12 Community Healthcare Networks (CHNs) within CHO DNCC.
Each CHN will deliver primary healthcare services across a population of approximately 50,000 with GPs involved in delivering services and a GP lead will be employed within each CHN.

The Community Healthcare Network (CHN) GP Lead will support the development and maintenance of relationships between GPs, GP practices and Primary Care professionals within their Network in order to ensure delivery of high quality, person-centred care to the local population (GP Agreement 2019).

The CHN GP Lead will support the Network Manager and CHN Management Team to roll-out clinical programmes; develop innovative solutions and multidisciplinary approaches to challenges within the CHN; and to implement CHN structures and service delivery within CHNs.
