Mellany McLoone is Chief Officer for CHO DNCC and has full responsibility and accountability for the delivery of all Community Healthcare services in the area.
Community Healthcare Services are the broad range of services that are delivered outside of the acute hospital setting.
DescriptionThe HSE Addiction Service responds to the presenting problem of substance misuse.
The service aims to meet the needs of this vulnerable cohort of the population by providing a stabilisation treatment service to the population for whom opioid misuse is/was a problem.
The Dublin North Addiction Service consists of eight dispensing sites – seven Addiction Treatment Centres (ATCs) and one residential stabilisation unit with a total number of 800 individuals attending these dispensing sites.
Within this service, each member of the pharmacy team works collaboratively as part of the multi-disciplinary team within the clinic site and reports directly to the Chief Pharmacist.
Pharmacists are responsible for the dispensing and supply of opioid agonist therapy (OAT) i.e.
methadone and buprenorphine as well as other directly observed therapy associated with their addiction.
These include prescribed psychotropic medication and periodically medication for infections, DVTs, Hepatitis C, and other health problems.
Anti-retroviral therapy may also be dispensed and supplied on-site for patients in some ATCs.
Medication is also provided from the pharmacies for take-home use.
Within the client group who attend the service for treatment, there are a number of individuals with co-addictions to a range of illicit drugs, prescription medications, and alcohol.
A significant number of people have complex needs.
The team works to improve social, housing, psychological, and other aspects of the lives of the client cohort by interventions within the team and with outside agencies in each location.
Key-working and care-planning are approved systems of care within the service.
The pharmacist should engage with other pharmacists working in the service and attend the monthly Pharmacy Team meeting.
Engagement within the team may require the provision of information to other pharmacists and preparing presentations on various topics and engaging with the development of Policies and Standard Operating Procedures from time to time.
