Nurse required for South Doc CIT in Cork/Kerry The post holder is an experienced nurse who, acting within their scope of practice, will provide quality care for patients in their home, usual place of residence or approved treatment location appropriate to patient needs.
They will demonstrate safe, clinical decision-making and expert care for patients.
The post holder will demonstrate critical thinking in the clinical decision-making process.
They will work collaboratively with the community intervention team to meet the needs of the patients while supporting the delivery of policy and procedures.
Some duties will include but not limited to: · Provide nursing services to patients within their own home, at GP premises/health centres or other community setting in liaison with other members of the team.
· Work in collaboration with Consultants/GPs, and other agencies to provide comprehensive packages of care.
· Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
· Prioritise, organise, and manage own workload in a manner that maintains and promotes quality.
· Advise CIT Manager of any circumstances which give cause for concerns in relation to patient care.
· Assess, implement, and evaluate treatment/interventions and care for patients.
· Deliver care according to the organisation's guidelines and evidence-based care.
· Ensure that agreed service levels are maintained.
· Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care in collaboration with the CIT Manager.
Nurses must be proficient in community nursing.
Full time and part time hours available with every second weekend.
10 hours shifts.
No overnights and latest roster time is usually until 23:00.
Full clean driver's licence essential.
