__________________________________________________________________________________________________________ Job Overview: We are currently seeking a dedicated and compassionate Outreach Support Person to join our team.
The successful candidate will provide supports within the framework of New Directions, an outcomes-based individualised approach to services for people with intellectual disabilities.
The purpose of the role is to support individual/s in an outreach service to access a wide-range of community-based services and opportunities to meet their needs and interests.
St. Aidan's Services are committed to the provision of services designed to provide for the preferences of the individuals we support. Staff therefore must carry out their duties with regard to the aims and objectives of St. Aidan's Services.
In this role, you will support individuals with intellectual disabilities in accessing a wide range of community-based services and opportunities, helping them achieve their personal goals and fostering their integration into the local community.
(Contracts are with the Service, this current project is supporting with an Outreach Service.
Permanent 30 hour contract) Key Responsibilities: Person-Centred Support and Care Planning: Assist in developing individualised care plans that reflect the unique needs, preferences, and goals of each individual.
Support individuals in setting and achieving personal goals, ensuring plans are regularly reviewed and adapted.
Promote individuals autonomy by facilitating informed decision-making and encouraging active participation in their own care.
Collaborate with individuals, families, and multidisciplinary teams to create holistic care plans addressing physical, emotional, and social well-being.
Community Integration: Support individuals in participating in community activities and accessing services to foster community inclusion.
Assist in building positive relationships between individuals and community members, advocating for inclusive practices Effective Communication and Advocacy: Communicate effectively with individuals, using tailored methods to suit individual needs, including assistive tools for those with cognitive or sensory impairments.
Listen actively to understand and respond to individuals concerns, needs, and preferences.
Advocate for individuals rights and needs, ensuring their voices are heard in all decisions.
Educate clients and families about available resources and support systems, empowering informed decision-making.
Empathy and Relationship Building: Demonstrate empathy and compassion in all interactions, building trust and fostering strong, supportive relationships with individuals.
Create an inclusive environment that respects the dignity and emotional needs of each individual, promoting well-being and positive outcomes.
Safeguarding: Remain vigilant for signs of abuse or neglect, taking immediate action to safeguard individuals according to protocols.
Maintain up-to-date knowledge of safeguarding policies and procedures, participating in regular training.
Work with multidisciplinary teams to address safeguarding concerns comprehensively.
Ensure all care decisions prioritise client safety, rights, and dignity, balancing transparency with confidentiality.
Risk Assessment and Management: Conduct regular risk assessments to identify potential hazards in the care environment, implementing preventive measures.
Respond promptly to incidents, following established emergency procedures to ensure safety.
Continuously monitor and review risk assessments, updating them as necessary based on individuals needs or environmental changes.
Educate individuals about recognising and reporting potential risks, fostering a proactive approach to safety.
Managing Behaviours of Concern: Identify early signs of behaviors of concern and potential triggers, using positive behavior support techniques.
Apply de-escalation techniques effectively to manage acute episodes of challenging behavior.
Work under guidance to implement appropriate intervention strategies, maintaining a safe and supportive environment.
Health and Safety: Ensure compliance with all health and safety regulations, including infection control, fire safety, and emergency procedures.
Report and document all accidents, incidents, or safety concerns promptly to the appropriate person or team.. ___________________________________________________________________________________________________________ Qualification Requirements:- A minimum of QQI Level 5 in Health Care Support/Social Care (A Diploma or Degree in Social Care or other relevant discipline advantageous).
A minimum of  six months experience of working with people with an Intellectual Disability/ASD required.
Must hold a full clean driving license and willing to drive company vehicles.
Possession of a valid driver's license and willingness to transport individuals using company vehicles.
Proficient in English language both oral and written Have the legal right to work in Ireland and currently hold valid work authorisation for Ireland Desirable & advantageous Criteria: Ability to speak Ukrainian and/or Russian.

 Demonstrated ability in relationship building, networking and promoting inclusion.
 Flexibility and ability to respond to changing needs and situations.
 Excellent organisational skills  Proven ability to work autonomously and on own initiative.
 Ability to collaborate and work as a member of a team.
 Be proactive, with high degree of motivation and drive.
 Strong organisational and time management skills.
 Good IT and administrative skills.
