Details of Service: The Public Health Nursing (PHN) and Community Nursing Service is a generalist nursing service underpinned by the principles and values of primary care and public health, including those of equity, accountability and evidence-based nursing care, working to public policy agenda and reform.

The enhanced community care (ECC) model is implemented in Cavan Monaghan and PHN and community nursing have played a significant role in this change model.
This includes the development of a Nurse led Community Intervention team and specialist teams for management of chronic disease.
Specialist nurses are core to the chronic disease management teams.
Public Health Nursing and Community Nursing Services are delivered within a range of settings: in the home, primary Care/health centres, schools, community health networks and chronic disease management hubs.
PHNs and community nursing staff work in collaboration and in partnership with other members of the primary care teams and also with acute / community hospital services, voluntary bodies and statutory and non-statutory agencies.

In line with Slaintecare, the HSE's plan to strengthen its governance and responsiveness to the needs of the population has taken a major step forward with the creation of the Health Regions which are tasked with a greater focus on integrated care and effective decision-making with a local focus on population health needs.
By ensuring that care is available and delivered to people who need it at the right time, in the right place and as close to home as possible.

Demand for care will continue to rise in 2025 and beyond, driven by population demographics – most significantly the increase in the number of people over 65 years of age which has risen by 22% over seven years.

Reporting Relationship: The Assistant Director of Public Health Nursing Enhanced Community Care Nurse Practice Development Coordinator (ADPHN ECC NPDC) will report to and be professionally accountable to the Director of Public Health Nursing.

Purpose of the Post: Under the leadership of the Director of Public Health Nursing, The Practice Development Co-ordinator (PHN Services) will

Facilitate the development of a continuous process of improvement in PHN and Community Nursing practice and excellence in the quality of care being provided. The Practice Development Co-ordinator (PHN Services) will strategically plan, develop and evaluate nursing care in collaboration with public health nursing management team and nursing staff Support the implementation of the enhanced community care model and the associated specialist nursing posts To support the integration of community healthcare services within the Networks in response to the needs and requirements of the population and requirements to deliver services. The role requires a combined knowledge of clinical practice, project management, service management, continuing nurse education and professional development and evaluation of outcomes.
The Practice Development Co-ordinator (PHN Services) will be required to participate in audit and research as determined by the role.

Eligibility Criteria Qualifications and/ or experience: 1. Professional Qualifications, Experience, etc. a) Eligible applicants will be those who on the closing date for the competition:

i.
Be registered, or be eligible for registration, in the Public Health Nursing Division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann).

AND ii. Have not less than 5 years satisfactory experience as a Public Health Nurse in the Irish Health Sector or in an analogous post in another jurisdiction, or an aggregate of five years such experience.

AND iii. Have successfully completed a post registration programme of study, as certified by the education provider, which verifies that the applicant has achieved a National Framework of Qualifications (NFQ) major academic Level 8 or higher award maintained by Quality & Qualifications of Ireland (QQI) or can provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 8 or higher standard in one of the following: Health Care, Quality Improvement, Risk Management, Health and Safety or Leadership and Management related area.

OR iv. Give a commitment to undertake a relevant post registration qualification at not less than Level 8 (QQI) qualification in one of the following: Health Care, Quality Improvement, Risk Management, Health and Safety or Leadership and Management related area within a specific time scale to be decided.

AND b) Candidates must possess the requisite clinical, leadership, managerial and administrative knowledge and ability for the proper discharge of the duties of the office.

2. Annual Registration (i) Practitioners must maintain live annual registration in the Public Nursing Division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnaimhseachais na hEireann).

And (ii) Practitioners must confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

3. Health Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

4. Character Candidates for and any person holding the office must be of good character.

Post Specific Requirements: Demonstrate depth and breadth of nursing experience as relevant to the role, including:

Implementation of quality improvement programmes, or evaluation and re-organisation of Public Health Nursing practice as relevant to the role. Experience of clinical practice management. Experience in the delivery of education or training. Other Requirements Specific to the post: Access to appropriate transport to fulfil the requirements of the role as this post will involve travel.
