Cpl Healthcare in partnership with our client, a non-profit organisation based in Dublin, Ireland, provides services for people who are homeless and at risk of homelessness.
The purpose of this role is to undertake individual work with families and customers who face chronic exclusion due to their complex and overlapping needs, utilising an Intensive Care and Case Management Model.
The role involves working collaboratively within an intensive family therapeutic support model, assessing needs, advocating on behalf of families, providing support, and making appropriate referrals to ensure a planned and effective exit from homelessness.
What you will do: Work within the framework of the overall objectives of the organisation.Work as part of a multi-disciplinary/multi-agency service.Engage families with their individual needs, considering dynamics relevant to their needs and behaviours.Work within a five-day service (Monday – Friday 9-5, 9-4 Friday).
Flexibility may be required to meet service needs.Develop and maintain links with the Health Service Executive, Tusla, Probation, and other services for effective communication and pre-discharge planning.Develop community links with accommodation and service providers to ensure collaborative working.Engage effectively with all stakeholders in the child's, parent's, and customers' lives, such as schools and extended family.Contribute to a case conferencing model ensuring effective liaison with external professionals and families.Provide one-to-one engagement to support families in making informed decisions.Contribute to the design and implementation of support packages for families.Implement agreed education programmes, utilising coaching and mentoring interventions.Contribute to the formulation of customer needs assessments and support plans, keeping up-to-date casework records.Work with customers to identify significant needs/behaviours affecting their situation.Attend and contribute to meetings and reviews as required.Implement the policies and procedures of the Project, including Health and Safety policies.Attend team and organisational meetings as required.Engage in regular supervision sessions with the Manager or Team Leader.Participate in reflective practice and relevant training and development courses.Undertake other duties as assigned in consultation with management.What you will need: 2 years experience working with high need families at high risk, utilising an intensive case management and family support model.BA in Social Care or Equivalent Qualification.Experience of working with customers who have co-existing conditions.Experience of team working in social care or clinical disciplines.Report writing abilities.Experience of liaising with other agencies.Understanding of issues pertinent to high need families with chronic child welfare issues and homelessness.Computer skills.Communication skills.Knowledge of children, adolescents, and issues affecting parents.Ability to plan and implement programmes.Flexibility.If you feel this role is the right fit for you, please apply via the link below or contact Joanna Wator directly at:
