Overview The post of CNM3 has a pivotal role in the effective co-ordination of patient discharges throughout the acute service and will provide leadership and co-ordination to all areas across Cavan Monaghan Hospital.
The post holder will be instrumental in designing and implementing discharge policies and procedures, ensuring the efficient and smooth transition of patients from the acute hospital to their homes or to other appropriate destinations such as rehabilitation, step down facilities or other relevant hospitals or residential facilities.
The CNM3 will be responsible for assessing and evaluating referrals in from other hospitals, coordinating the multidisciplinary team (MDT) to make discharge decisions while monitoring key bottlenecks in the discharge process to support efficient discharge planning at ward level.
The post holder will lead the discharge team to ensure efficient discharge processes and plans.
The post holder will also deputise for Head of Beds as and when required.
Essential Criteria 1.
Statutory Registration, Professional Qualifications, Experience, etc (a) Eligible applicants will be those who on the closing date for the competition: (i) Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland (NMBI)(Bord Altranais agus Cnimhseachais na hireann) or entitled to be so registered.
And (ii) Have at least 5 years post registration full time (or an aggregate of 5 years post registration full time) experience of which 2 years (or an aggregate of 2 years post registration full time experience) must be in the speciality orrelated area.
And (iii) Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
And (iv) Candidates must demonstrate evidence of continuous professional development.
And (b) Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.

Annual registration (i) On appointment, practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnimhseachais na hireann).
And (ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

Health Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

Character Candidates for and any person holding the office must be of good character.
Post Specific Requirements Demonstrate depth and breadth of experience in Acute Medical/Surgical Nursing to include experience of discharge planning as relevant to the post.
