Registered Advanced Nurse Practitioner/Advanced Nurse Practitioner Candidate (Anp/Anpc) Accelerated Care Pathway (Emergency Department)

Details of the offer

The Accelerated Care Pathway Advanced Nurse Practitioner (ANP) in the Emergency Department (ED) plays a pivotal role in enhancing patient care and optimising resource utilisation.
The primary purpose of this role is to swiftly assess patients upon their arrival to the ED, determining the clinical pathway for each individual.
By identifying candidates who can be effectively managed through outpatient services, outreach programmes and other ANP pathways within the hospital, the ANP significantly contributes to reducing unnecessary admissions.

The ANP will also employ advanced clinical judgment and assessment skills to perform post triage assessment of patients, ensuring that those who require immediate inpatient care are prioritised.
Early identification of patients needing admission is crucial; the ANP will initiate timely assessments and treatment plans, thereby accelerating the decision-making process.
This proactive approach not only enhances patient outcomes but also minimises delays, ultimately reducing both the ED and inpatient length of stay.

Additionally, the ANP will collaborate closely with interdisciplinary teams, including ED and clinical teams, nursing staff, and allied health professionals, to ensure seamless patient transitions between services.
This collaboration will support the development of care pathways that are responsive to the needs of the patient while aligning with best practices in emergency care.

By focusing on effective patient management and strategic decision-making, the ANP will play a crucial role in improving patient flow, enhancing satisfaction, and promoting a high standard of care within the ED.
This innovative approach aligns with our commitment to delivering efficient, patient-centered care while fostering a culture of collaboration and excellence within the healthcare team.
