RGN required for Complex Case Management Team under Cork/Kerry Community Healthcare • Staff Type: RGN • Location: Based in Westfield Integrated Care Centre Ballincollig –involves driving between different sites (to Cork University Hospital, Mercy University Hospital , occasionally to University Hospital Kerry and around community settings in Cork) • Details: The CCMT (The Complex Case Management Team) is a community based service that was established to assist acute hospitals (Cork University Hospital, Mercy University Hospital, University hospital Kerry , occasionally Bantry and Mallow hospitals) with case managing complex patients aged between 18 and 65 to facilitate a discharge from acute hospitals.
The RGN will support existing team and have a caseload of patients where will be required to identify barriers to discharge and scope and solution discharge plans through integrating with acute and community services.
• Start Date: Mid-September • Period: Ongoing • Shift Days: Monday – Friday hrs per week) • Shift Times: – Requirements: Transportation.
Experience with discharge planning and /or working with complex patients and families desirable.
