Senior Social Worker- Part time, Specified Purpose Contract Reports To -Director of Clinical Supports Key Relationships & Interactions Internal Clinical Team- Directors of Clinical Supports, Healthcare co-ordinators, Safeguarding co-ordinator, Assistant Psychologist.
Day Services Programme Co-ordinators, Local Managers & Leaders and Frontline staff supports.
External Families and Natural supports; Health Services Executive; Citizen Information services, Primary Care teams.
Terms & Conditions We are seeking an energetic, flexible, innovative, and enthusiastic person to join our Clinical Team to support the development of a targeted Social Work service, which will complement the range of supports available to the people we support.
The current vacancy is a Specified purpose contract.
The Salary for the post is on a pro rata basis on the Senior Social Worker grade of the HSE 2021 pay scale.
The location of the work may vary within the Dublin & Kildare area.
Working week: the standard working week applying to the post is 23.4 hours per week requiring post holder to work flexibly as per the service.
Professional Social Work supervision provided internally by Director of Clinical Supports.
Job Overview The Senior Social Worker, under the supervision of the Clinical Director, will lead the development of social work supports across day and supported living services.
The post holder will establish a clear prioritisation system for social work intervention and input into holistic assessments and interventions.
The post holder will play an active role in the clinical team and will support local teams to identify, assess and escalate risk to appropriate internal and external bodies.
The post holder will establish a clear onwards referral and signposting system for appropriate community supports.
Individual assessments, and the provision of a range of advisory, practical, and skilled emotional supports for adults with a disability and their circles of support will be a cornerstone of the work.
The Senior Social worker will carry a defined case load, support social care teams to work with natural supports and provide mediation supports in complex areas.
Principle Duties and Responsibilities Plan, deliver and engage in systemic interventions as appropriate with individuals, families, groups, organisations and communities.
Promote the Will and Preference of the Service User in planning and decision-making processes.
Establish a mechanism for identifying and prioritising cases where holistic Social Work assessments and interventions are required.
Promote independence, self-reliance, self-determination and empowerment with persons in their environment, with families and local groups.
Make it possible for service users to advocate for their own needs, or where appropriate advocate on behalf of service users.
Develop Social Work case management systems and structures across WALK service.
Develop information and signposting for people we support, staff and families.
Develop frontline social care staffs capacity to navigate community services and make appropriate onwards referrals.
Identify where social work service can collaboratively work with other disciplines / agencies to benefit the lives of the people we support and their families.
Assess where social conditions are a major factor in health and social wellbeing, consult and plan with the service user/ relevant team/ service and arrange appropriate social services for those who need them.
Develop and disseminate referral pathways and procedures with other health care professionals.
Responsible for the development of a family leadership network.
Attend meetings, circles of support, clinical reviews and other relevant fora as required.
Implement models of best practice / evidence- based practice.
Incorporate Social Work values and ethical principles in planning, developing, implementing, and reviewing interventions.
Develop Social Work role in the service and build strong community connections with HSE and Community partners.
Take an active role in an appropriate level of planned professional supervision.
Engage in reflective practice and support other groups to incorporate reflective practice skills in their work.
Identify and escalate risk to the relevant internal and external bodies.
Develop training modules which actively supports staff to engage with wider circles of support.
Key Outcomes People supported have models of support that are optimal to meeting their needs and achieving meaningful self-determined socially inclusive lives.
People supported are helped to develop strategies to cope and overcome difficult situations.
Robust circles of support are developed and nurtured with individuals at risk of social isolation and or other identified needs.
People are supported to advocate for relevant supports and services.
Frontline staff teams are supported to work more effectively with the individuals they support.
The Organisation benefits from a more holistic viewpoint when planning and delivering supports.
Essential Criteria to be assessed by CV Must be registered in the Social Work Register maintained by the Social Work Registration Board maintained at CORU.
Must have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the role.
A relevant third level qualification in Social Work Minimum of 3 years post qualification experience.
Demonstrate values in line with the organisations human rights-based approach to service delivery.
Experience of working in complex case management environments Experience of supporting staff teams Knowledge and experience of National Standards and Regulations.
Knowledge of Assisted Decision-Making Legislation.
Have a full drivers license.
Desirable Criteria - to be assessed by CV Demonstrated ability to support individuals with complex needs in community settings.
Prior experience of establishing or developing social work service Project management skills Supervisory or management qualification.
