Job Description: As a Social Worker in our Mental Health team, you will work with adults who are experiencing a wide range of mental health conditions.
You will play a vital role in assessing, planning, and delivering care and support to improve their well-being and independence.
Working closely with healthcare professionals, families, and community resources, you will ensure that our service users receive the holistic care they need.
