Social Worker - Permanent Part-Time Co. Clare Heathcare Direct Healthcare Direct employs 66 HSCPs that include Chartered Physiotherapy, Occupational Therapy, Speech & Language Therapy and Dietetics.
Healthcare Direct provides multidisciplinary interventions to over 130 Nursing Homes and a full programme to support the orthopaedic service in two private hospital groups.
Healthcare direct is a key enabler of integrated post-acute rehabilitation, offering bespoke therapy interventions to a broad range of patients.
The provision of post-acute assessment, case management, rehabilitation and reablement will be led by HCD therapists under the governance structures established by Mowlam Healthcare in partnership with the HSE and other providers.
Background to post Healthcare Direct is strategically developing integrated, age friendly rehabilitation capacity in post-acute rehabilitation and home care, building on its expertise and track record as Irelands biggest private provider of long-term residential care.
This vision is in keeping with the move towards an AFHS and the HSE strategy to address give priority to urgent and emergency care for older adults.
This service will reflect the standards and principles underpinning rehabilitation as set out by the World Health Organization (WHO 2023), HSE national clinical programmes and contemporary evidence on rehabilitation (EU 2019).
This will integrate with existing Enhanced Community Care developments (Community Health Networks, Community Specialist Teams, Older People and Chronic Disease) and age friendly pathways under development in acute hospitals.
It is proposed that this mixed economy approach will reflect the positive lessons from the EU Observatory (2024) which highlights the benefits to the population when public and private capacity is combined.
Key functions Healthcare Direct have a highly experienced team of Physiotherapists and Occupational Therapists supported by Physiotherapy and Occupational Therapy Managers.
We specialise in providing healthcare services to the public and private sectors as well as Occupational Health services to employees of companies and organisations right across Ireland.
We provide patient centred quality care in a wide range of settings including nursing and residential care settings, community hospitals, acute inpatient wards, specialist care centres, private company premises and also private sessions in patients homes.
As such, we see a broad and diverse range of clinical presentations.
Due to significant business growth, we are currently looking to employ a Social Worker to join our team in Clare, initially on a part-time basis with potential to increase hours based on candidate availability and caseload growth.

Working as a Social Worker in our team will involve: Patient Assessment: Evaluating patients needs, including mental health, social, and financial situations.
Counselling: Offering emotional support and guidance to patients and families to help them cope with illness, injury, or trauma.
Care Coordination: Collaborating with healthcare professionals to create and implement patient care plans, including discharge planning and rehabilitation support.
Resource Referral: Connecting patients and families to community resources, such as financial assistance, housing, mental health services, or support groups.
Crisis Intervention: Providing immediate support during medical crises, including end-of-life care or critical diagnoses.
Advocacy: Ensuring patients rights and access to healthcare services, while advocating for their needs within the healthcare system.
The post holder will require the following: CORU Registration A broad range of under-graduate clinical experience with Elderly care being desirable but not essential Strong written and verbal skills Will require own transport, clean drivers license and a Smart Phone Healthcare Direct fully believes in supporting its staff members, and has supported a number of employees through further development courses.
