Are you a Professionally Qualified Social Worker with CORU registration and experience working with adults who have complex needs?
Medforce have an opportunity available for a CORU registered Social Worker to join a well-established team in North County Louth.
Position: Social Worker Location: North County Louth, Ireland Service: Adults Disability Service Contract Type: 6 months initially with a view to extend, 35 hours per week, Monday - Friday Salary: Increment in line with public sector payscales The Service: Our client runs a dedicated multidisciplinart team of professionals within the Adults Disability Service in County Louth who are committed to providing high-quality support, advocacy, and care to adults with disabilities.
The goal is to empower individuals to live as independently and inclusively as possible, ensuring their rights and dignity are respected at all times.
Position Overview: The Social Worker will work with individuals who have a wide range of disabilities, their families, and other support services to develop comprehensive care plans and ensure the highest standard of care and support is provided.
This role involves offering professional social work interventions, assessments, and advocacy to assist service users in achieving their full potential and maintaining a good quality of life.
Key Responsibilities: Assessment and intervention for adults with disabilities Provide support and guidance to service users and their families, promoting independence and improving quality of life.
Advocate for individuals, ensuring access to services, entitlements, and resources.
Collaborate with multidisciplinary teams including healthcare professionals, occupational therapists, and service providers to coordinate comprehensive care.
Maintain accurate records and reports in line with legal, ethical, and organisational standards.
Conduct home visits, liaising with families and caregivers as necessary.
Support individuals through complex care transitions, ensuring they are well-informed and supported.
Keep up-to-date with legislation, policies, and best practices relating to adult disability services.
Participate in regular supervision and professional development to ensure the highest standards of practice.
Essential Criteria: A recognised Social Work qualification and registration with CORU.
Previous experience working with adults with disabilities or in a similar setting.
Strong knowledge of Irish legislation and regulations relevant to disability services.
Excellent communication, advocacy, and interpersonal skills.
Ability to work both independently and as part of a multidisciplinary team.
A full, clean driving licence and access to a car for work purposes.
