Location:
Caislean Services operates a range of services including day, residential, respite, training, education, employment opportunities, Individualised services, and leisure activities which are geographically dispersed though the West Sector Adult Services. Qualifications/Experience/Skills
A current Live Register with NMBI (An Bord Altranais), RNID qualification is an advantage.Candidates must have a current full clean manual driving licence as this is an essential requirement of the post.Candidates must have relevant experience and an appreciation of the responsibilities of the post and issues affecting the everyday lives of individuals with an Intellectual Disability.Good judgement, discretion and confidentiality.The ability to communicate clearly, sensitively, and effectively in both written and verbal forms, and to communicate with family members and work with them for the best outcome for the person supported.Creative problem solving skills.The ability to work independently while recognising when to seek support from interdisciplinary colleagues as appropriate.Patience, flexibility, creativity and eagerness to become involved in a range of activities.The ability to educate and upskill colleagues and family members in positive health promotion needsThe successful candidate will be required to co-operate in a team environment with a person-centred philosophy, participate in person-centred planning, individual reviews, team meetings, attend and participate in training courses, and actively contribute to team based performance management.
Working Hours
75 hours a fortnight based on a 14 day/night duty roster. The successful candidate will be required to work weekends, public holidays and night duty to meet service needs The Brothers of Charity model of service is based on Personal Outcome Measures requiring employees to be flexible in their working hours to provide a quality service delivery for each individual.
Annual Leave
24 days per annum. Appropriate service related leave will be granted after 5 and 10 years service respectively.
Remuneration
Department of Health salary scales 01/01/2024:
Staff Nurse - €35,068 per annum x 11 increments - €51,263 (MAX) per annum. Long Service Increment (LSI) - €52,790 per annum is granted after 3 years on the maximum of the scale.
Enhanced Nurse - €41,952 per annum x 7 increments - €52,948 (MAX) per annum. Long Service Increment (LSI) €54,499 per annum is granted after 3 years on the maximum of the scale.
The successful candidate will be granted incremental credit subject to satisfactory verification of previous relevant nursing service. Additional payments will be made for weekends, public holidays, and sleep-ins and night duty when worked. The Brothers of Charity Services model of service is based on Personal Outcome Measures requiring employees to be flexible in their working hours to provide a quality service delivery for each individual.
Tenure
This post is Permanent full-time and pensionable.
Probation
A probationary period of nine months from the date of appointment applies to the post.
The employment may be terminated at any time during the probationary period should the
employer find that the appointee is unsuitable to continue employment. The probation period
may be extended at the Employers discretion.
Job Description
The Brothers of Charity model of service is based on Personal Outcome Measures requiring employees to be flexible both in their approach and their working hours, in order to work with people supported to achieve their hopes and dreams.
Specific Conditions
Staff must have a positive attitude towards working with persons with an intellectual disability and help to ensure that they lead as normal and enjoyable a life as possible.
Staff must treat each person with an intellectual disability as an individual and at all times acknowledge and respect the rights and personal dignity of the person with an intellectual disability.
Staff are required to establish and maintain relationships with adults with disabilities that are based on respect and equality and that promote their independence.
Main Duties And Functions
Develop a good working relationship with families and key people involved with the people supported, as appropriate.
Actively pursue and ensure the delivery of a person centred service, through the delivery of Personal Outcomes. Be involved as a Key worker and assist new staff in this role. Record and monitor outcomes and track progress.
Actively support the implementation of the National Interim New Directions Standards.
Record outcomes and track progress
Be aware of and take responsibility in ensuring compliance with standards as outlined by the 2007 Health Act and HIQA Standards.
Formulate and implement programmes designed for the needs of the individual, in co-operation with the multidisciplinary team as required.
Take responsibility for the supply, storage, stock and administration of medications in accordance with the policy of the Service.
Take part in the education and training of support workers in responsible and safe medication management and carry out practical assessments of their competence in their area of work.
Be responsible to ensure that hygiene and infection control standards are actively enforced.
Ensure full compliance with all Covid-19 precautions in place.
Assist people supported with personal banking and adhering to the National Guidelines on managing individual finances.
Devise appropriate programmes of activity for the individual in line with the expressed or felt needs and desires of each individual.
Keep appropriate records to enable full reports on people supported to be available including individual person plans, personal outcomes assessments and reviews.
Promote community integration and maximum participation for the individual.
Supervise and direct the work of other staff including Support workers and Housekeeping staff and provide feedback and direction as required.
Ensure that each person supported money is spent according to their wishes and all that appropriate records are kept of same in line with policy. Awareness of resource management is also important with the ability to achieve efficiencies in resource usage
Co-operate on all aspects of programmes with the staff team and the multidisciplinary and other stakeholders to maintain, develop and enhance the established partnership model.
Ensure that a good working relationship is fostered and maintained in the work place.
Embrace change and take on new developments in I.T. around storage of records and inputting of data onto databases
Facilitate reporting at Individual Plan meetings and at service user review meetings.
Be conscious of Health & Safety matters in the workplace and, in particular, to comply with employees' obligations as set out under the Safety, Health & Welfare at Work Act, 2005.
Ensure that all accidents/incidents to people supported by the services, staff or visitors or equipment are reported promptly and that all staff are made fully aware of the contents of the Safety Statement for the Centre. Record all incidents/accidents as per the AIRS system.
Ensure that high standard of hygiene is maintained in the houses, by carrying out household duties such as cooking, cleaning, and laundry work. Be responsible to ensure that hygiene and infection control standards are actively enforced.
Operate within the policies and procedures of the Services at all times.
Ensure confidentiality is maintained in all matters concerning people supported including the safe storage of reports and records.
Promote dignity at work and show respect to the people we support, colleagues and stakeholders in the course of duty.
Take an active role and lead out Team Based Performance Management Programme in your assigned area.
Participation in hydrotherapy activities is an essential component of the post.
Facilitate persons supported with holidays and breaks away.
As the duties and responsibilities of any post in the Services are likely to change with the ongoing need of the people supported by the services, i.e. age, level of disability, Personal Outcome Measures etc., staff are expected to have a high level of flexibility, willingness and an ability to develop new approaches to their work as the service users needs demand.
Organise staff cover as required.
Act up for the Team Leader if required.
Perform such other duties appropriate to the post as may be assigned from time to time by the Team Manager/Deputy. The duties must be carried out in person at all times.
Enhanced Nurse Additional Duties And Responsibilities If Applicable
The following outline the additional roles and responsibilities incumbent on you in your new role as an Enhanced Nurse and are in addition to the duties that are outlined in your job description for your current nursing role.
To practice Nursing according to the Code of Professional Conduct issued by the Bord Altranais agus Cnimhseachais na hireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines;To work within your scope of practice and take measures to maintain and enhance the competence necessary for professional practice;Complying with and adhere to all relevant policies and procedures;Promoting the safety, health, welfare and social wellbeing of persons supported and respect the dignity and confidentiality of persons supported and their families;Promoting persons supported advocacy and maintain persons supported privacy and dignity;Performing the full range of clinical duties appropriate to your area of practice and as determined by your line manager to ensure a comprehensive nursing service is delivered to the persons supported you work with;Maintaining appropriate and accurate written/electronic nursing records, person centred plans and care plans regarding person supported support in accordance with local / national / professional guidelinesUtilising appropriate evidence-based and recognised screening tools, risk prediction models, assessment and enhanced skills to support person supported in their home (e.g. Early Warning Scores, Falls Assessment);Supporting and cooperating with the sharing of tasks as per national agreements;Engaging in continuous skill acquisition and be supported as appropriate in doing so to ensure the ongoing development of the enhanced roleTaking responsibility for own competency and learning and development needs and actively contribute to the learning and development of the wider nursing, multi-disciplinary and primary care teamCompleting all mandatory training as deemed necessary by the Director of Nursing, the employer and Bord Altranais agus Cnimhseachais na hireann (Nursing Midwifery Board Ireland);Supporting the training and education of nursing students and participating in the clinical/ workplace induction of new nursing and clinical support staff (e.g. Care Assistants and other relevant grades in the ID Sector).Contributing to the identification of training needs pertinent to the clinical area;Developing clinical teaching skills and participating in the planning and implementation of orientation, training and teaching programmes for nursing students and other healthcare staff allocated to the service e.g. Preceptorship, mentoring and coaching.Co-operating with and implement, where appropriate, strategies developed to move work to the community, including decongregation of settings;Co-operate with changes arising from implementation of the recommendations of the review of nursing and non-nursing in all areas including areas and changes arising from the implementation of national policies and legislation.Participating in clinical governance structures within the local / regional / national clinical governance framework;Reporting complaints/ incidents and assist with the investigation of same as appropriate;Facilitating and supervise the delegation of appropriate tasks to other clinical support grades as part of the nursing team;Co-operating with the implementation of the agreed new structures developed to support Integrated Care Organisations and Government policy in the sector.Supporting the implementation of all recommendations from the HCA review including development of appropriate policies and guidelines to promote and sustain delegation;Co-operating with the introduction of ICT systems including those to support the Safe Nurse Staffing Framework, or any agreed alternative as may apply in the ID sector, and input, utilise and analyse the data;Collecting and input data on nursing metrics or other metrics relevant to persons supported;Ensuring that records are protected and managed as per HSE / local policy and in accordance with relevant legislation;Working closely with colleagues across health services in order to provide a seamless service delivery to the persons supported as part of integration;Participating in and contributing to and providing data as required to any verification process at local, regional or national level for the purposes of documenting and assuring delivery of the range of duties specified in this Contract;Undertake any other duties in accordance with your scope of practice as may be assigned by the Assistant Director of Nursing, Clinical Nurse Manager II or Employer.
The role of the post holder will not be limited by reference to this Job Description. It is expected that the role will evolve as professional, structural and service user demands change, with the post holder being expected to demonstrate flexibility skills that will facilitate this.
