Support Worker - Children'S Disabilities

Support Worker Childrens Disability Team The Support Worker will work as part of an interdisciplinary team to deliver a quality service to children and their families who are attending the service.
The support worker will report to the Childrens Disability Network Manager.
The CDNTs focus on providing integrated services to children with complex needs, so the support worker role involves both care and developmental support.
Responsibiilites of the Support Worker.
Assist with review of clinical reports from private providers, documenting the outcomes and recommendations in a standardised template under the supervision of the Children's Disability Network Manager.
Assist with transfer / referral to other services where appropriate under the supervision of the Children's Disability Network Manager and clinical team.
Assist with the discharge of clients from the CDNT under the supervision of the Children's Disability Network Manager and clinical team.
Assist with file reviews and preparation of clinical note summaries under the supervision of the Children's Disability Network Manager and clinical team.
Assist in the administration and day to day operation of the service as needs arise.
Research and prepare resources for universal family support and strategies.
Participate in team meetings and report to the Children's Disability Network Manager on matters affecting the delivery of service.
Maintain a high standard of documentation, including service user files in accordance with local guidelines, the principles of confidentiality, the Freedom of Information (FOI) and GDPR Acts.
Contribute to the development and implementation of information sharing protocols and audit systems.
Contribute to the ongoing development of the service in keeping with good practice and HSE objectives.
Maintain professional standards in relation to consent, confidentiality, ethics and legislation.
Observe all rules relating to Health and Safety and Conduct at Work and use any equipment provided in a safe and responsible manner.
Report any incident or potential incident which may compromise the health and safety of others and take appropriate action.
Report any accidents, near misses to the person in charge and ensure completion of incident/ near miss forms.
Have a working knowledge of the Health Information and Quality Authority (HIQA) -+ Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc.
and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role.
Support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service.
Education and Training Level 6 qualification in a relevant field, such as Health and Social Care, Social Care, Childcare, or Special Needs Assistance (SNA).
Working within multidisciplinary teams (which might include speech and language therapists, physiotherapists, and psychologists) is a plus.
CDNT support workers often collaborate closely with various professionals to provide holistic care.
Participate in team-based development, education, training and learning.
