The Clinical Nurse Specialist (Respiratory) will be part of the multidisciplinary team involved in the development of the COPD Outreach service in County Wexford.
The COPD Outreach Service consists of Early Supported Discharge, Assisted discharge, Admission avoidance and case management of patients with COPD as outlined in the Guidance document for setting up COPD Outreach Services for Healthcare Professionals (National Clinical Care Programme (2020).
The service provides a 'Hospital at Home' programme for patients who present to hospital with an uncomplicated acute exacerbation of COPD.
The aim of the programme is to provide early supported discharge to patients with a confirmed and uncomplicated diagnosis of COPD, so that they can be discharged within 72 hours of presentation to the hospital.
Suitable patients are discharged home under the care of the outreach team with governance from the Respiratory Consultant for approximately two weeks.
These patients receive approximately one to three home visits over the next 14 days to monitor their progress.
Home visits consist of an assessment and monitoring of the patient.
It includes education on inhaler techniques, support with new equipment such as oxygen, home BiPAP and nebulisers, education on COPD, teaching self-management skills and early intervention strategies.
The person appointed to this post will work in the COPD Outreach Service which is a developing service within the overall respiratory service of Wexford General Hospital and County Wexford community services.
Wexford General Hospital is a Level 3 acute general hospital, in the HSE Dublin and South East Health region, which is academically affiliated with University College Dublin.
The hospital serves a population of approximately 150,000.
The post holder will work as part of a multidisciplinary team, led by a Respiratory Consultant, which includes a Clinical Specialist Physiotherapist delivering co-ordinated, evidence based, patient-centred care for patients with COPD, within the boundaries of County Wexford.
