Salary: Appointment will be made at CNM 2 at a point in line with Government pay policy

Whole Time Equivalent Hours: Part time/ across a 7-day roster (including Out of Hours)

Position Summary The Nursing Administration CNM2 will play a key role in the hospital, providing operational and clinical leadership out of hours in respect to patient flow, admissions over weekends and safe patient care.
The candidate is responsible for the maintenance of a high standard of patient care in line with HIQA standards which is achieved through safe staffing levels, effective communication pathways professional education and development of all nursing staff, acting as an expert practitioner, participating in the formation of organizational objectives linking nursing with its goals and managing resources.
To participate in and co-ordinate (out of hours) Emergency Response Teams to Fire, security alerts, cardiac arrest and clinical emergencies.

Principal Duties and Responsibilities: CLINICAL FOCUS: The role will encompass another key area but not limited to the following

· Provide patient flow management from the acute hospitals (weekend admissions)

· Administration of patient admissions to the hospital out of hours.

· Arrange medications for new admissions

· To work closely with clinical staff to implement hospital admission and discharge policies.

· Work closely with hospital Clinical Nurse Managers, Patient Flow departments and Consultants in referring hospitals to generate and manage referrals.

· Liaise with external hospitals and referral agents to ensure appropriate admissions who would benefit from rehabilitation at Clontarf Hospital.

· Liaise with Medical Teams, Ward Nursing Team and On-call Physiotherapist in event of weekend admission.

· Maintain a positive working relationship both within the hospital and with external hospitals and agencies.

· Maintain and monitor the 'waiting list' and manage all patients' referrals from acute hospitals.
Have the ability to critically analyse each referral to ensure the patient's meet the criteria for admission.
Request additional documentation if required to support the referral e.g.
physiotherapy, Occupational Therapy, Social Work reports etc.

· Work to ensure full occupancy of beds in the hospital including private occupancy.

· Regular timely reporting of bed and resource utilisation to senior management and circulating on a regular basis reports for key clinical decision makers to resource utilisation.

· Coordinate the daily bed management meeting with senior nurse management.

· Liaise with general administration to ascertain if patient's insurance cover is appropriate for admission to private rooms within the hospital if required.

· Analysing, monitoring and forecasting trends in respect of admissions.

· Update the hospital bed management system.

· Direct and active participation in service planning, implementation and evaluation and in overall resource management of the department, including continuous quality audit and performance.

· Ensuring the application of best practice including the generation of action plans to continuously improve the effectiveness of the bed management function.

· Create a culture of accountability, openness to change, respect for the individual and teamwork that supports the ethos of the hospital.

· The CNM2 will ensure the patient is cared for in a safe environment and hygiene standards are maintained.

Act as a resource for nursing staff as appropriate. Assist ADON in the implementation of change and service development initiatives. · Liaise closely with the Infection Prevention and Control CNM both within the hospital and outside the hospital to ensure standards are met.

· Work closely with all members of the MDT, to promote the effective, timely and safe discharge of patients from the hospital i.e.
home or to community services etc.

· Coordinate and assist in the smooth transfer of patients who need to return to the acute hospital for further treatment or in the case of a failed rehab.

· Comply with the hospital's confidentiality and GDPR policy.

· To act as a patient/family advocate through the application of ethical, legal and professional knowledge and skills.

· Create a culture of accountability, openness to change, respect for the individual and teamwork that supports the ethos of the hospital.

· To maintain effective relationship management with all staff and external agencies/hospitals.

· Complaint management

· Act as a resource for nursing staff as appropriate and provide expertise.

· Ensure all acutely unwell patients triggering EWS system are identified and escalated.

· Carry out regular Senior Nurse Management Rounds

QUALITY, AUDIT & RESEARCH · Understand the importance of quality in relation to every day work and participate in quality system measures to ensure that services are provided and an agreed quality standard.

· Analysing, monitoring and forecasting trends in respect of admissions and hospital operations.

· Participate in and encourage the development of evidence-based practice.

· Assist in audits and support metric data collection

· Initiate, evaluate and communicate action plans as a result of audit to all nursing staff

· Monitor incident occurrence on the unit.
Evaluate and initiate action plan if required.
Communicate all finding to the ADO

· Ensure all staff comply with policy in relation to incident management

· Facilitate others in the development of audit tools to measure outcomes of nursing practice.

· Patient satisfaction must be of prime concern, every patient is to be treated as an individual and provided with high quality service in terms of courtesy, kindness, interest and efficiency.

· Actively seek Patient feedback and communicate this to the ADON

· Communicate and liaise with Nursing Quality Department and the QSRM department

· Understand the importance of providing an excellent Customer Care Service to all patients, consultants and colleagues to uphold the mission, vision and values of the hospital.

· Ensure the application of best practice including the generation of action plans to continuously improve the effectiveness of the bed management function.

· Participate fully in all Health and Safety related hospital initiatives and policies.

· Direct and active participation in service planning, implementation and evaluation and in overall resource management of the department, including continuous quality audit and performance.

· Discuss present performance and future needs with the Director of Nursing.

· Participate in writing of policy and procedure documents at hospital level

· Contribute to investigation of adverse occurrences, medication incidents and patient complaints.

GENERAL RESPONSIBILITIES AND RESOURCE MANAGEMENT · Support the function of the Nursing Administration Office

· Support Policy Development

· Sit on appropriate multidisciplinary committees and sub-groups within the Nursing and at hospital level that are concerned with initiating practice development projects.

· Ensure that the hospital's health and safety, fire and other statutory regulations are adhered to.
Observe and comply with Hospital Policies and Procedures and Health and Safety regulations.

· Be aware of all policies in relation to health and safety at ward level especially

- Fire Prevention

- Moving and Handling

- Waste Management

- Risk Management

· To participate in and co-ordinate (out of hours) Emergency Response Teams to Fire, security alerts, cardiac arrest and clinical emergencies.

· Assist with the requisitioning, monitoring, storing of and use of supplies including general stores, medications and linen.

· Use equipment efficiently and properly to promote good care and prevent waste.

· Liaise with Maintenance Department to ensure maintenance and upkeep of equipment.

· Complete staffing rosters and deploys as required.

· Monitor and manage sick leave and other leave in accordance with policy

· The CNM2 will liaise with relevant management teams in relation to Cleaning Assistant, Portering and Catering Assistants and support out of hour sick calls.

· Work with HOD's to ensure smooth out of hours operation of the hospital.

PERSONAL & PROFESSIONAL DEVELOPMENT · Take responsibility for own professional and personal development updating.

· Be pro-active in developing own competencies in a structured way linked to regular professional, personal development planning.

· Practice within the Clontarf Hospital policies and procedures, Scope of Professional Conduct, and Scope of Nursing Practice.

· Undertake appropriate further training and education to enable you to practice within a variety of clinical medical/surgical areas and enable you to support the service needs within the scope of professional practice.

QUALIFICATIONS & EXPERIENCE · Possess a recognised general nursing qualification.

· Be registered in the General Division of the Register of Nurses kept by NMBI

· Have at least five years post-registration general nursing experience most of which has been in an acute general hospital.

· Competent in Admission and Discharge Planning

· Have experience in Gerontology

· A degree level education is essential.

· Completed a Management Course or commitment to undertake a relevant management course.
