____________________________________________________________________________________________________ Job Details: Community Inclusion Manager (SCL Grade) We are seeking applications from energetic and enthusiastic individuals to join our team for the above position.
Candidates must be highly motivated, creative and experienced with a passion for engaging with individuals on a daily basis.
Candidates must be self-driven, focussed and person-centred.
As the Community Inclusion Manager, your role will focus on leading and supervising a team of carers, as well as managing the day to day co-ordination of the day service unit.
The successful candidate will ensure supports are delivered within the framework of New Directions, with an outcome-based individualised approach to services for people with intellectual disabilities.
The purpose of the role is to ensure individuals have access to a wide-range of community-based services and opportunities to meet their needs, interests and desires.
(Contract is 30 hours per week on Social Care Leader payscale) (All permanent contracts are with the service.
This current assignment will be leading our new day service which is scheduled to open in the last quarter of 2024) Key Responsibilities: 1. Management: - Leads and manages day services from an operational perspective working with the Assistant CEO; supporting the strategic development of day services.
Ensures services are delivered in an effective person-centred manner, meeting the needs of the people supported; ensures that the Philosophy, Ethos and Core Values of St Aidan's Services is put into practice, encouraged and sustained. Provides supports within the framework of New Directions, an outcomes-based approach to supports and services for people with intellectual disabilities.
Effectively lead, motivate and support day service leads and staff ensuring all new staff are inducted and developed into their role through effective supervision and training. Leads & Manages Day Services, representing the service on external Steering Groups/Committees/Working Groups; manages admissions & discharges.
Liaison person between other services/ schools and St Aidan's Services with regard to school leavers/ new referrals who have expressed an interest in sampling/ attending our services.
Carry out an assessment of support needs to identify supports required to ensure effective services can be provided to a new referral if approved.
Put in place a transition plan for the individual transitioning from children's services to adult services/ an individual transitioning from another service provider to St Aidan's Services.
Attend referral/ transition meetings and collaborate with individuals and their family/ guardians to develop an individualized support plan.
Carry out the role of Safeguarding Designated Officer DO - To attend DO training, to achieve DO certification, to be a member of the DO team, providing DO support to the organisation.
Ensures that services are delivered in a cost-effective manner, within the resources available to the organisation; ensures that units/leads operate within allocated staffing & financial resources. 2. Client Supports: - To ensure that each person is treated with the utmost respect and dignity and that the Service continually strives to ensure a "good life" for each person supported.
To promote a Human Rights Based Approach to working with the people we support.
Ensuring that the individual, or their decision support maker, are actively involved in the decision-making process To actively seek new interests and opportunities for the people we support to develop meaningful roles in their communities.
To manage and co-ordinate supports for individuals with accessing work experience/ employment opportunities through job coaching and mentoring.
Emphasis is on making links and connections within the community to increase opportunities for the individual being supported.
To ensure the best possible health and wellbeing of the people we support in all areas of their lives including health and personal care.
To oversee and ensure skills development and confidence building of the people we support is being implemented in the day services.
To be responsible for overseeing that the individuals care & support plans are prepared, implemented and updated in line with New Directions.
Liaise with the multidisciplinary team and other agencies as is relevant to the individual's care/support needs and constantly review the quality of service delivered to the people we support.
To ensure that the lead of each unit keeps you informed of the individuals care and welfare and advises you of any concerns that may arise.
To Promote capacity building within the organisation with a view to applying the least restrictive practices where deemed appropriate.
To ensure that there is a pleasant atmosphere for the people we support within day services.
Ensures that there are effective systems and practices for the people we support and family/carer consultation and involvement in service planning and review.
3. Compliance & Regulations: - Ensures the roll out of National Policies across the Service; including the implementation of the standards in line with New Directions and management of the Easi-Tool .
Ensures all services are delivered in a manner compliant with all applicable legal and regulatory requirements including all laws, regulations, codes of practice, standards, national policies etc (HSE, Legislation, Health & Safety Authority etc).
Ensures that policies & procedures are being adhered too; ensure consistent practices across day services.
Co-ordinates internal audit practices and approach, to assess and enhance compliance.
Submit KPIs and Metrics to the Senior Day Service Manager to facilitate analysis of service delivery, performance management and compliance across day services.
Manage service complaints and service issues constructively and effectively.
Ensures that there are effective risk management systems and procedures operating in day services.
Ensures that there is an effective Health & Safety regime operating across all activities, services and facilities.
Ensures all relevant records and documentation are kept appropriately and up to date in relation to the people we support and staff in particular ensuring compliance with record keeping in line with legislation.
Ensures that all prescribed medicines, drugs and treatments are administered to individuals attending the service, that the required records are kept and updated. Storage and administration of drugs and medicine complies with St. Aidan's Policies.
To ensure that all Safeguarding concerns are dealt with in a prompt and timely manner 4. Staff Management: - Delegate appropriate responsibility and authority to the level of staff under his/her supervision and consistent with effective decision making whilst retaining overall responsibility and accountability for outcome.
To be responsible for ensuring that the work carried out by staff meets the requirement of the care plan and is being delivered in a safe and supportive manner considering the will and preference of people we support.
Manages the activities of managers to ensure the effectiveness and efficiency of services delivered across day services.
Promotes and monitors an effective staff performance management and development process across day services.
Ensures there is an open and timely communication process across day services and residential when required.
To identify, prioritise and address training and development needs within the day service.
Supervise, direct and support the development needs of assigned staff.
Facilitate training for staff as required, including refresher training as part of a continuous professional development programme Ensures workforce planning, rostering and staff performance is of impeccable standards so as to meet the requirements of the individual's needs.
____________________________________________________________________________________________________________ Qualifications & Experience Requirements :- Hold a Degree in Social Care, Applied Social Studies, Applied Social Studies in Social Care or foreign equivalent (validated by QQI) Hold a management qualification (advantageous) Several years' experience in Disability Services.
Highly experienced Social Care Professional with a minimum of 2 years experience in a supervisory capacity Hold a full clean driving licence & willing to drive company vehicles if required Have the legal right to work in Ireland and currently hold valid work authorisation for Ireland ____________________________________________________________________________________________________________ Required Skills & Attributes: Specialist knowledge of, and expertise in the Intellectual Disability sector.
Thorough understanding of current thinking, approaches and developments in ID services delivery.
Detailed knowledge of all relevant legislation/regulations/standards and a proven ability to implement standards and regulations.
Experience of strong relationship building with key sector stakeholders: HSE, people supported/guardians.
Evidenced ability to reorganise and re-structure services to deliver enhancements and efficiencies.
Excellent leadership and motivation skills.
Effective interpersonal and communication skills Drive and commitment to service values.
