Senior Medical Social Worker

Excel Healthcare is currently recruiting a dedicated Senior Medical Social Worker for a position based in an acute hospital in Navan, Meath.
This is a contract role for full-time or part-time hours.
Responsibilities of this Senior Medical Social Worker job Deliver quality services per CORU and HSE guidelines.
Carry out psychosocial assessments with patients and families Support patients in relation to the emotional and practical aspects of their illness and, where appropriate, advocate on their behalf.
Be responsible for the management of the day-to-day provision of the social work service under the direction of the Principal Medical Social Worker.
Conduct home assessments and follow up visits where necessary.
Attend and contribute to weekly multidisciplinary meetings.
Ensure the delivery of social work services in accordance with legislation, policies and procedures, guidelines and protocols.
Develop effective working relationships with patients, families, different agencies, community resources and other professionals.
Keep accurate records of all social work interventions, including statistical data in relation to client work.
